ENROLLMENT

If your child will soon be entering school, start planning ahead for the enrollment process. 

You will need the following documents: 

• CERTIFIED STATE birth certificate

•  Shot record (complete)

• Physical / Healthcheck (must be within the year)

• DENTAL exam

HEALTH NEEDS

If your child has any special health needs, you will need to contact your school nurse to obtain information needed for school enrollment.  There are specific forms that your health care provider will need to complete if your child needs specialized health care procedures (including medication administration) while at school. 

You can contact the school nurse for the school your child will be attending or contact the Director of School Health Services if you have additional questions.

 REQUIREMENTS for ENROLLMENT

Dental Exams and HealthCheck Physicals are required for:

• First time entrants (PK or K)

• 2nd Graders 

• 7th Graders

• 12th Graders

• First time entrants (PK &/or K)

• 6th & 11th graders for 7th and 12th grade entry.
