Registering as a Vendor with Boone County Schools
BASIC VENDOR REGISTRATION
Vendors interested in doing business with Boone County Schools (BCS) must first submit a completed registration packet to the BCS finance office for approval. All active and prospective vendors must, at a minimum, provide BCS with a completed Vendor Registration Memorandum and IRS W-9 Form. Many vendors will also be required to provide BCS with a valid West Virginia Business Registration Certificate as issued by the West Virginia State Tax Department and proof of their registration with the West Virginia Secretary of State. Vendors may qualify for an exemption from the business registration certificate and secretary of state filing. Available exemptions are detailed on page two (2) of the Vendor Registration Memorandum.

Completed vendor registration documents are to be submitted to registration.boone@k12.wv.us. Once BCS finance personnel have reviewed and approved the submitted registration packet, an email will be sent back to the vendor confirming registration.
Please note that if the option at the top of the vendor registration memorandum is marked by the vendor, an assertion is being made that the business does not qualify for an exemption from one or more of the registration requirements. Therefore, a completed registration packet must include (1) the Vendor Registration Memorandum, (2) completed IRS Form W-9, (3) WV Business Registration Certificate, and (4) proof of Registration with the WV Secretary of State's Office.
If the second option near the middle of the vendor registration memorandum is marked by the vendor, an assertion is being made that the business qualifies for one or more filing exemptions as detailed on page two (2) of the vendor registration memorandum. The exemption(s) being claimed must be included in the appropriate field of the vendor registration memorandum. Vendors are responsible for understanding those registration exemptions and providing all documents that are not covered by the exemption(s) being claimed.
BOONE COUNTY SCHOOLS IS NOT PERMITTED TO CONDUCT BUSINESS WITH VENDORS WITHOUT A COMPLETED AND APPROVED VENDOR REGISTRATION.
CONTRACTUAL AGREEMENTS
If a vendor seeking to do business with Boone County Schools will be providing products or services under a contract, memorandum of understanding, or other type of agreement, that business must also complete and attach the following documents to their agreement prior to it's execution:
Manual changes to the Agreement Addendum may not be made unless agreed upon by the Boone County Superintendent of Schools prior to signing of the agreement.
BOONE COUNTY SCHOOLS IS NOT PERMITTED TO ENTER INTO AGREEMENTS WITH VENDORS WHO REFUSE TO COMPLETE THE BCS PURCHASING AFFIDAVIT, BCS NO DEBT AFFIDAVIT, AND ADDENDUM TO VENDOR'S STANDARD CONTRACTUAL FORMS.
